Instructions for Filing an Intent to Graduate Form
- Obtain a graduation packet, which includes the Intent to Graduate Form, from the Cashier's Office on any campus
(hours vary) and pay the $25 graduation fee. Check catalog and handbook for deadline dates.
- Complete the personal data section at the top of the form.
- Career/Technical (AAS degree) students need to visit the Placement Office (GN-105) at
the Pellissippi campus or call (694-6559) to set up an appointment to complete a placement file and have the Intent to
Graduate Form date stamped.
- University Parallel (AA/AS degree) students need to verify that all transfer credit has
been accounted for by reviewing transcript on P.S..Web or with advisor.
- Make appointment with your advisor to discuss requirements and review form prior to the last day of
early registration.
- Advisor: Complete "Degree Applying For" section (left side of form) and the
"Transcript Evaluation" section (top right side of form). Attach the following to the back of the form:
On-Course audit (screen 681); specific page articulation agreement, if applicable; schedule of courses that
complete program (screen 109), and any paperwork on approved course substitions/waivers or other exceptions.
Forward Intent to Graduate form and additional documentation to your Department Head or Program Coordinator
for verification and signature. Inform student of graduation status.
- Department Head/Program Coordinator: Verify "Transcript Verification" section
(right side of form) then approve as submitted with signature or do not approve and discuss problems with
advisor (also state reason on form). If approved, forward signed form to the Admissions & Records Office.
Inform advisor of graduation status. Be sure student is notified by the advisor of any problems concerning
graduation requirements.
- During the first week of classes next semester, you may verify with Admissions & Records Office (694-6561)
that your Intent to Graduate Form was received by the deadline.If you have outstanding obligations to the College
or other problems that would prevent your graduation, the Records Office will notify you. NOTE: Students jeopardize
their graduation status by changing their final semester course schedule.Consult with your advisor about any changes.
- Register for the state-required general education exit test (CBASE) through P.S...Web, or by submitting a
registration form to the Admissions & Records Office. Dates, times, and section numbers are provided in
this packet. EXCEPTION:You are exempt from taking the CBASE exit exam if a prior degree has been earned or if
English is your second language. Call the Testing Center Office (694-6454) if you have questions.
Student Loan Recipients: (Federal Stafford Loans) need to complete an exit loan counseling session.
