Instructor Feedback Form - Interpreter Performance

A part-time contract sign language interpreter for SSWD has been place in your classroom to facilitate communication between you and a deaf or hard of hearing student. To ensure quality interpreter services please answer these questions fairly and honestly and submit to Ann Satkowiak Director, Services for Students with Disabilities, asatkowiak@pstcc.edu, 539-7153.

  1. Do you feel confident the interpreter will not disclose private information that was obtained during the interpreted assignment, whether it regards you or the students?
  2. Do you believe interpreted communication occurred fluently without distracting errors that caused embarrassment or confusion?
  3. Do you believe the interpreter respected professional boundaries and did not interfere with, participate in, or bring unwarranted attention to him/herself during the interpreted assignment?
  4. Do you believe the interpreter understood the course subject being taught well enough to interpreter its concepts effectively?
  5. Do you believe the interpreter arrived to class on time prepared to work, and that the interpreter was professionally or appropriately dressed?
  6. Would you work with this interpreter again? If no or maybe, please explain why.
