Services for Students With Disabilities (SSWD) does not request documentation from providers. It is the student's responsibility to request, obtain, and submit disability documentation to support requests for accommodations.

For medically-based disabilities, students should submit the Medical Documentation Form (RTF, PDF) to their provider(s) for completion. Completed forms signed by the provider should be returned to SSWD.

Students with a psychological or emotional disability should submit a Psychological Form (RTF, PDF) to their provider(s). Completed forms signed by the provider should be returned to SSWD.

SSWD follows specific documentation guidelines for students with Learning Disabilities (LD), AD/HD, Autism Spectrum/ Asperger's, and other cognitive disorders (RTF, PDF). The student or provider should submit documentation to SSWD.

*Please note that the student/patient is responsible for any costs pertaining to copies of released medical or psycho-educational records.
