How to request accommodations and services for disabilities at Pellissippi State

You are advised to begin these procedures before you graduate from high school (at least 3 months before you plan to enroll at Pellissippi State) in order to make sure you have time to complete everything before classes begin.

  1. Submit your PSTCC application for admission to the PSTCC Admissions office and pay the $10 application fee. Visit the Admissions website at http://www.pstcc.edu/departments/admissions/. Phone (865) 694-6568.
  2. Submit your high school transcript (copy of your grades) and ACT scores to the PSTCC Admissions office.
  3. Contact the SSWD office to complete disclosure of your disability, accommodation request forms and have current documentation of your disability sent by your diagnostician (school, psychologist, doctor, etc.)
  4. Please be sure your documentation meets PSTCC guidelines. We have requirements that may be DIFFERENT from your high school requirements. The documentation requirements can be found on our website at: http://www.pstcc.edu/departments/swd/guidelines1.html
  1. Learning Disability (LD) and Attention Deficit Hyperactivity Disorder (ADHD) documentation must contain a complete recent (within 3 years) psycho-educational evaluation and report complete with IQ testing on an adult scale and recent achievement testing.   2. Medical, sensory, and psychological/psychiatric disorders must have a form from our office completed by the appropriate diagnostician within the last year. Forms can be downloaded from our website at http://www.pstcc.edu/departments/swd/guidelines1.html
  3. Make an appointment to meet with a staff member in the SSWD office to complete your accommodation plan to give to your professors.
