What do I do to get services and/or accommodations for my disability at Pellissippi State (PSCC)?

Once a student is admitted to PSCC:

  1. Complete and submit to SSWD the following forms:

    *All forms are provided in Rich Text Format (RTF) or Portable Document Format (PDF). PDF files require Adobe Reader.

    • Disclosure Form (RTF, PDF)
    • Release of Information Form (RTF, PDF)
    • Accommodation Planning form (RTF, PDF)
  2. Submit to SSWD disability documentation that meets PSCC Guidelnes:

    • Learning Disabilities (LD), AD/HD, Autism Spectrum/ Asperger's, and other cognitive disorders (RTF, PDF).
    • Psychiatric/Psychological impairments (RTF, PDF)
      (bi-polar disorder, schizophrenia, dysthymia, and others)
    • Mobility impairments (RTF, PDF)
      (health/medical)
    • Deafness/Hearing impairments (RTF, PDF)
      *Must include an audiogram or other medical information indicating the type and degree of hearing loss.
    • Visual impairments (RTF, PDF)
      *Must include information regarding visual acuity and type/degree of vision loss.
  3. Meet with an SSWD staff member

    Students will be notified when appropriate documentation is on file and may choose to schedule a meeting with SSWD staff to arrange accommodations and/or services. An academic accommodation plan will be established. Students are responsible for delivering accommodation plans to individual instructors. Accommodation plans must be updated each semester.
