Professional Services for Deaf and Hard of Hearing

Welcome to SSWD Professional Services; we are happy to assist you during your academic pursuit. Once it has been determined you are eligible to receive professional services, an Academic Accommodation Plan will be created. The accommodation plan will include either a Sign Language Interpreter or a TypeWell transcriptionist. To receive services:

• Complete disclosure form and submit with medical documentation
• Complete the Professional Service Request form (Word, PDF) and submit with class schedule at least 2 weeks in advance. NOTE: services cannot be guaranteed without sufficient notice
• Inform SSWD Coordinator of any schedule changes immediately
• Read, sign, and comply with the appropriate Student Responsibility form below:
  • TypeWell Services (Word, PDF)
  • Interpreter Services (Word, PDF)
• Inform SSWD Coordinator more than 24 hours in advance when you will be absent. NOTE: habitual no-shows and last-minute cancellation are unacceptable and services may be suspended as a result
• Complete the Professional Service feedback form (Word, PDF) at the end of the semester
