This may include contacting patients via telephone, email and/or direct mailings to stress the clinical importance of following the established treatment plan; Validates data for various disease registries and conveys this information to the Care Coordination team; Assess clinical information via the electronic medical record for case finding regarding referrals to care coordination; Identify SMG patient’s with the potential for high risk complications and coordinate the appropriate referral to the care coordination team; Assist the Regional RN Team Leader/Social Worker with the coordination of institutional facilities as necessary to facilitate access to physical health and/or behavioral health services needed by the SMG patient and to help ensure the proper management of the patient’s acute and/or chronic physical health or behavioral health conditions;Assist the Care Coordination team with the implementation and follow up of the care plan; For transition care, if appropriate, will facilitate the coordination of services necessary to safely transition the patient to the community, including, but not limited to, member needs related to housing, transportation, availability of caregivers, and other transition needs and supports; Act as an advocate for an individual's health care needs by identifying and communicating any barriers to a safe transition and strategies to overcome those barriers;With guidance from the Regional RN Team Leader and assistance from the social worker, facilitates patient understanding of the physician’s treatment plan, including but not limited to medication adherence, preventive care, and self management skills; Act as an advocate for an individual's health care needs by identifying and communicating opportunities for care intervention that have been established by the Regional RN Team Leader, including identifying and addressing gaps in care; Utilize criteria for authorizing appropriate home and community based services, obtain authorization for those services, if needed, and confirm those services are being provided and that members' needs are being met; Maintain appropriate and ongoing communications, as necessary, and collaborations with patients, seeking guidance from the assigned Regional RN Team Leader should any changes/modifications be needed in the treatment plan; Assist with record attainment and ensuring coordination of SMG patient’s primary care provider, specialists, and other providers and care programs to facilitate the comprehensive, holistic, person-centered approach to care; Working under the direction of the Regional RN Team Leader, assess, through follow up and monitoring, the patient’s status, needs, and progress; if progress is static or regressive, convey this information to determine reason and proactively encourage appropriate adjustments in their plan of care, providers and/or services; Report quantifiable impact, quality of care, and/or quality of life improvements as measured against the care coordination goals; Establish working relations with referral sources, community resources, and care providers.
EDUCATION: Graduate of board approved Licensed Practical Nursing program. EXPERIENCE: Minimum of 2 years of experience in clinical or administrative role in medical organization. Prior experience with quality reporting, disease management, population health management, or other related health care environments. Experienced in Windows and Microsoft. This position is responsible for assisting with both clinical and front office tasks as directed by the manager, physician(s), or mid-level provider(s) in the care of patients. Working hours are 10AM-7PM and Saturday rotations with hours from 8AM-12PM. Examples of Duties: (List does not include all duties assigned): Front Office Greets patients and visitor in a prompt, courteous, and helpful manner; Assists patients with ambulatory difficulties, or other needs, as appropriate; Checks-in patients, verifies and updates necessary information in the medical records/database, performs all registration functions and collects copays and balances due as appropriate; Answers and screens telephone calls, takes messages, and provides appropriate information to phone callers;Maintains computerized scheduling and follows office scheduling policies; Clinical Escorts patients to exam rooms and prepares them for examinations and procedures according to the physician/mid-level preferences; Records vital signs of patients, including pulse, blood pressure, height, weight, neck size, BMI, etc. in the Electronic Medical Records (EMR) system; Performs CPAP (Continuous Positive Airway Pressure) mask fittings, desensitization, and other adjustments as requested by the clinician, etc.; Assists the providers in conducting patient education and contacts patients regarding test results, CPAP downloads, overnight pulse oximetry, or other results as directed; Handles phone nursing when appropriate, to include triaging, pre-certs, referrals, prior authorizations for prescriptions, calling in prescriptions, etc.; Uses software program to download CPAP compliance reports as well as creates custom reports and other related requests as directed by the providers; Serves as contact for DME (Durable Medical Equipment) companies for order requests, insurance related issues, and CMN's as necessary; Maintains stocked, neat and clean exam rooms as well as common work areas on a daily basis; Adheres to site-specific protocols and expectations as well as company policies and procedures (including the corporate compliance program); Follows state and federal regulations, such as OSHA and HIPPA guidelines; Strong verbal and written communication skills; The ability to work effectively with physicians, patients and other staff; Experience in basic clinical procedures, such as phlebotomy, injections, EKGs, holter monitor, etc. as needed.
Education: High School Diploma or equivalent required, prefer additional vocational or college credits. Experience: A minimum of six (6) months experience in a similar clinical setting required.
