Apply for Services

What do I do to get services and/or accommodations for my disability at Pellissippi State?

 

1. Complete and submit to Disability Services the following forms:

Disability Services does not request documentation from providers. It is the student's responsibility to request, obtain, and submit disability documentation to support requests for accommodations. Please note that the student/patient is responsible for any costs pertaining to copies of released medical or psycho-educational records.

GUIDELINES FOR DOCUMENTATION:

  • Learning Disabilities (LD), AD/HD, Autism Spectrum/Asperger's, and other cognitive disorders (RTFPDF).

  • Psychiatric/Psychological impairments (RTFPDF)
    (bi-polar disorder, schizophrenia, dysthymia, and others)
    *Please note that the student/patient is responsible for any costs pertaining to released medical or psycho-educational records.  
  • Medical/Mobility impairments (RTFPDF)
    (health/medical)

  • Deafness/Hearing impairments (RTFPDF
    *Must include an audiogram or other medical information indicating the type and degree of hearing loss.

    *Must include information regarding visual acuity and type/degree of vision loss.
    *Please note that the student/patient is responsible for any costs pertaining to released medical or psycho-educational records.  

3. Meet with a Disability Services staff member.
Students will be notified when their files are complete and may choose to schedule a meeting with Disability Services staff to arrange accommodations and/or services. An academic accommodation plan will be established. Students are responsible for delivering accommodation plans to individual instructors. Accommodation plans must be updated each semester.
